NDIS plan reviews and reassessments
Needs change. When your plan no longer fits, we document what's different and help you make the case for the right funding.

How we help
Review your current plan
We look at what is funded, what you're using and where the gaps are.
Identify changed needs
Updated functional assessments that show clearly what has changed and why more or different support is needed.
Prepare the evidence
Progress reports and recommendations written to the NDIS's reasonable-and-necessary criteria.
Support at the meeting
Help preparing for the reassessment conversation, and advocacy where it's wanted.
The reassessment process
Review your plan
Understand what's in your current plan and where you need more support.
Collect evidence
Reports from therapists and specialists describing your changed needs.
Request the reassessment
Contact your planner or Local Area Coordinator and explain why.
Prepare
Be ready to talk about goals, barriers and how extra supports would help.
Attend the meeting
Present your evidence and be clear about the difference the supports will make.
Plan review questions
What's the difference between a plan review and a reassessment?
A plan reassessment (previously called a full plan review) looks at your whole plan and can be requested by you or started by the NDIA. A change of circumstances or a review of a specific decision are narrower processes. We'll help you choose the right one.
When should I ask for a reassessment?
When your disability or health has changed, your supports no longer meet your needs, your goals have changed, your living situation has changed, or you are facing new barriers.
What evidence helps?
Up-to-date therapist progress reports, functional capacity assessments and letters from treating professionals that describe the change in your needs and the supports required.
Is your plan still the right fit?
Call us, book online or send an enquiry. We'll talk through your goals, check your NDIS funding and match you with the right therapist.
